Although each chronic condition has unique symptoms and treatments, the lifestyle challenges can be very similar. Primary care physicians are instrumental in helping patients address these challenges. However, many health systems and medical practices hire specially trained nurses and social workers as community-based health managers (CBHM) to help patients manage their chronic conditions. CBHM services include:
Coordinating care between primary care physicians, specialty physicians and hospital staff
Assisting with medication management and ensuring patients take medication as directed
Answering questions and clarifying care instructions
Scheduling and prioritizing medical appointments
Providing referrals for services such as nutrition counseling and support groups
Identifying needs for home care
Transportation
While these are the top challenges for people managing chronic health conditions and ways in which a primary care practitioner or CBHM can assist, there are dozens of other opportunities for patients to get the assistance they need when working with these professionals.

About Us
Where We Come From + Where We’re Going
Let’s change the world of chronic illness.
Suddenly being barely able to climb out of bed and spending more time in and out of hospitals than at NYU was a confusing jolt into the world of chronic disease.
Everything about it was complicated, frustrating, and time-consuming. I was misdiagnosed—common with chronic illnesses that often share or mimic each other, or are too rare to be recognizable by a primary care physician. I had to manage my own case and even saved my own life by researching and advocating for a treatment my medical team had never heard of. Something didn’t add up.
Regardless of how someone with chronic disease feels living with 10, 20, or more debilitating symptoms on any given day, they are expected to carry on—with work, family, and social obligations. In a society that associates disability primarily with visible ailments or injury, those with an invisible illness—though they experience pain on a constant basis–are seen as lazy, full of it, or weak.
The truth couldn’t be more opposite.
In reality, only a handful of registered and qualified medical professionals can provide the care we need for the chronically ill in the U.S. With a surplus of medical resources in one country and shortages in the U.S., creating pathways to balance and benefit all sides seemed obvious. Acryno To helps level the playing field and equalize access to and distribution of these resources.
Beyond the platform, we create space to share and discuss the full chronic disease experience. Our podcast, Lions Rawr, was born of a need for the chronically ill community to speak, grieve, and learn.
No one living with chronic disease deserves to be put through the standard of care available in the U.S. today, or give up on their dreams as a result of it.
Thank you for being here. I’m so glad you’ve found the Acryno To family.
I created Acryno to help people get the best possible care.
